🏥 Navigating Transitional Care: Challenges and PerspectivesExploring the intricate landscape of transitional care from hospital to cardiac rehabilitation during the pandemic, this study delves into the experiences of older adults and healthcare providers. The qualitative design uncovers six key themes, revealing gaps in follow-up, emotional support, information provision, and decision-making spaces. The findings highlight crucial aspects for improving transitional care services in the post-hospitalization phase, especially for older adults after a cardiovascular event.📖 Read the full article: https://lnkd.in/gu8KmXbm#Healthcare #TransitionalCare #CardiacRehabilitation #PatientExperience #HealthcareProviders #PandemicChallenges #patientexperience

Ely struggled to breathe as soon as she was born.At just six days old, she was admitted to the Alberta Children’s Hospital and diagnosed with basal meningeal encephalocele, an obstructive mass of tissue and fluid growing in her nasal cavity.Due to her size and fragile state, surgery would need to wait until Ely was bigger and stronger. However, she would need intervention to help her breathe — at 29 days old, she received a tracheostomy. While Ely’s parents were relieved their sweet baby’s condition had stabilized, they were also overwhelmed. A child with trach requires 24/7 care by someone equipped to handle the medical emergencies that may arise. They needed to learn basic day-to-day trach care – how it works, how to keep it clean and unobstructed, and what Ely’s breathing and function were like at baseline. They also needed to learn more stressful things, like changing her trach, how to suction fluids out and what to do if it becomes blocked or dislodged, leaving Ely without oxygen. The KidSIM Pediatric Simulation program at the Alberta Children’s Hospital was an invaluable resource for them. Funded with community support, KidSIM is a simulation program for medical experts, families, and schools to practice real-time, emergency scenarios on high-tech, life-like mannequins. Care providers can keep their skills as sharp as possible, and families like Ely's learn how to care for their child.Learn more about KidSIM and how it helped families last year in our 2023 Report to the Community. https://ow.ly/aQTM50QfIU3

Meet Kay, Manager of Telemetry/Progressive Care Unit (PCU) at Rose (pictured 4th from the left). In this unit, they take care of patients with a variety of cardiac-related conditions: heart attacks, heart failure, post-pacemaker, post-stent, stroke, and more. "My favorite part is the team on my unit. We have a great group of nurses, PCTs, cardiologists, and hospitalists that work well together to ensure the patients have the best possible outcomes."Kay is proud of the Telemetry department because of the team's flexibility and adaptability. "We take care of all kinds of patients who came in for another reason, but their heart started acting up while they were here! This makes us well-versed in [many] patient diagnoses."We asked Kay what questions she receives from patients most often: ❓ How do I know if I need to come back to the ER? "If you have sustained chest pain or shortness of breath. Always better to be safe than sorry."❓ How did this happen? I eat healthy and exercise! "Family risk factors are a big cause of cardiac disease. Even those with a healthy lifestyle may have genetic conditions that are unrelated to diet and exercise."Happy Heart Month! How can Rose's cardiology program help you? https://bit.ly/3P0MVpw. #AmericanHeartMonth #HeartHealth American Heart Association

New research shows that older adults in "distressed communities" were 43% less likely to engage in cardiac rehab than those in "prosperous communities" (Distress Communities Index). Unfortunately - access gaps continue to plague our country, even on proven standards of care. Regardless of the level of community distress, similar benefits of reduced mortality and decreased hospitalizations were achieved with any engagement in cardiac rehab. Allowing patients to do cardiac rehab from home – rather than traveling to a hospital or rehab center could make a huge difference in participation rates. What to do? Support legislation to help level the playing field for those in rural and disadvantaged communities.
